Your monetary donation helps support Legacy of Life Hawaii’s mission to save and heal lives through organ and tissue donation. Your contribution will help us inspire and advance donation through public education and outreach. There are three contribution options:

Make a One-time Gift

If you would like to make a donation in honor of, or in memory of, someone, Legacy of Life Hawaii will send a personalized tribute card to the person or family indicated in the form below to notify them of your thoughtful donation, without revealing the amount of the donation. We will also send you a letter confirming your gift and notifying you that the tribute card has been sent to the person or family you indicated.

Endowment Gift (form needs to integrate with Bloomerang)

The Livingston M.F. Wong, MD, FASC Endowment Fund for Public Education and Outreach has been set up to educate Hawaii residents about the critical need for organ donation and the importance of registering as an organ/tissue donor. Educating Hawaii residents about organ and tissue donation is an important step toward building a healthy and caring community that embraces the true meaning of leaving a legacy of life to those in need.
