Hawaiʻi Donor Enrollment Day Registration Form

Hawai‛i Donor Enrollment Day is a virtual event with one goal: register as many kamaʻāina as possible as organ, eye and tissue donors in a single day. The observance brings together individuals and organizations to help educate communities on the importance of donation, and by encouraging willing individuals to join the Hawai‛i Donate Life Registry.

Prizes will be awarded to teams and individuals who have registered the most individuals. All registered participants will be entered into a drawing for a $100 gift card.
